Each applicant must appear in person with application.

Certified Copy of Birth.
This cannot be the hospital issued copy. It must be a State or County issued certificate. You may also use Naturalization Documents. Expired passports are acceptable for adult applications.

Identification.
You will need a State Issued driver’s license or identification card. If you do not have proper identification, you may have someone over 18 who have known you for at least two years complete an Affidavit of Identifying Witness. That person must present proper identification.

Photographs:
There are certain size and quality specifications. Please contact us before using photographs other that professional passport photos.

Children 15 years of age and younger must be accompanied by both parents at time of application. Each parent will need to provide identification. If both parents are unable to accompany their child at time of application a “Statement of Consent” must be completed, signed, notarized and presented at time of application.

Expedite Service is up to 3 weeks processing after mailed from Roseau County Treasurer’s office. It’s an extra $60 and added to the US Dept of State fee.

RENEWING A PASSPORT BOOK
(Less than 15 years old from the issue date)

Please submit the following:

• The DS-82 form
• Your recent US Passport Book
• Two recent color photographs (see #4)
• Fee- $75 payable to US Dept of State (If requesting expedited service it’s an additional $60, for a total of $135)

You may also purchase a passport card with the renewal of the passport book for an additional $20. The total of the fee would be $95.
